Description: On Monday, 01/08/2024, from approximately 1:00 p.m. to 4:30 p.m. \(UTC\), some users experienced message processing delays and overall slow performance in their PhishER platform. This incident was caused by an unexpected increase in traffic. Once we increased our compute capacity to manage the additional traffic, PhishER returned to normal performance by 4:30 p.m. \(UTC\). To prevent this type of issue in the future, we have increased our traffic capacity to make the affected PhishER features more scalable. No data loss occurred as a result of this issue.

Description: On Wednesday, 12/13/2023, from approximately 7:30 p.m. to 9:35 p.m. \(UTC\), some users experienced issues loading training modules in the KMSAT Learner Experience \(LX\) and ModStore. This incident was caused by a backend storage system upgrade on the KMSAT US console instance that did not include a certain access permission for training modules. As a result, users on the US instance of KMSAT were unable to access training modules. Once we added the proper permissions, users on the US instance of KMSAT were able to access training modules, and KMSAT began to return to normal performance. However, shortly after resolving the incident, we discovered some users were still experiencing issues loading specific training modules. After this discovery, we identified and rectified another permissions issue related to the same storage system. After completing the second fix, KMSAT returned to normal performance by 9:35 p.m. \(UTC\). Users who still experienced issues after this point were provided instructions on how to do hard refreshes to correct any remaining module loading issues. No data loss occurred as a result of this issue.

Description: On Tuesday, 12/5/2023, from approximately 7:30 p.m. \(UTC\) to Wednesday, 12/6/2023, at 2:30 p.m. \(UTC\), some users experienced delays when provisioning users in KMSAT. This incident was caused by an incorrectly configured file that was deployed during an update to our user provisioning service. Due to this issue, some KMSAT accounts were unable to sync with their identity providers. Once a fix for the file was deployed, all syncs resumed as previously scheduled, and KMSAT was able to return to normal performance by 2:30 p.m. \(UTC\). To prevent this type of issue in the future, we are improving our monitoring processes to ensure that the user provisioning service remains available. No data loss occurred as a result of this issue.

Description: On Thursday, 11/16/2023, at 2:30 PM \(UTC\) until Monday, 11/20/2023, at 7:00 PM \(UTC\), some KMSAT admins could not generate reports with custom date ranges. This incident occurred after deploying a fix to a separate bug related to reports and date ranges. After identifying the cause of the issue, we reverted the previous bug fix and the report behavior returned to normal performance. To avoid similar issues in the future, we will add more date and time zone testing to our testing process. No data loss occurred as a result of this issue.

Description: On Tuesday, 11/14/2023, from approximately 1:30 p.m. to 3:30 p.m. \(UTC\), some users experienced issues loading their KMSAT Account Settings page. This incident was caused by a backend update that deployed on Monday, 11/13/2023, at 3:47 p.m. \(UTC\) and unexpectedly affected the frontend of some KMSAT accounts. Once the issue was resolved, KMSAT was able to return to normal performance by 3:30 p.m. \(UTC\). To prevent this type of issue in the future, we are making adjustments to our updating processes. No data loss occurred as a result of this issue.
