Description: On Monday, 10/16/2023, from approximately 8:30 a.m. until 9:40 a.m. \(UTC\), some users experienced login issues and degraded performance in their KMSAT consoles. This incident was caused by higher-than-expected traffic on the database. Once we adjusted our database resources, KMSAT was able to return to normal performance by 9:40 a.m. \(UTC\). To prevent this type of issue in the future, we are making adjustments to our database resource settings. No data loss occurred as a result of this issue.

Impact: Minor | Started At: Oct. 10, 2023, 1:40 p.m.
Description: On Monday, 10/9/2023, from approximately 8:23 a.m. until 4:40 p.m. \(UTC\), some users on the EU instance of KMSAT experienced degraded performance in their consoles. This incident was caused by a background job that led to high database CPU usage. Once we paused the background job and deployed a fix, KMSAT was able to return to normal performance by 4:40 p.m. \(UTC\). To prevent this type of issue in the future, we are making improvements to our internal processes. No data loss occurred as a result of this issue.

Description: On Monday, 10/2/2023, some users on the EU and US instances of KMSAT experienced degraded performance in their consoles. These console issues happened from approximately 8:45 a.m. until 10:15 a.m. \(UTC\) for EU instances and 1:30 p.m. to 1:56 p.m. \(UTC\) for US instances. This incident was caused by a large number of requests due to high traffic. We quickly implemented additional resources to handle these requests and paused some nonessential services. Once this issue was resolved, all KMSAT instances returned to normal performance. Then, we resumed the nonessential services gradually until all services could return to normal functionality by 9:00 p.m. \(UTC\). To prevent this type of issue in the future, we are implementing additional improvements to handle high traffic more efficiently. No data loss occurred as a result of this issue.

Description: On Friday, 9/8/2023, from approximately 1:45 p.m. until 2:45 p.m. \(UTC\), some users experienced issues when accessing their training in the Learner Experience \(LX\). This incident was caused by a backend update that prevented some KMSAT queries from processing. Once the update was reverted, KMSAT was able to return to normal performance by 2:45 p.m. \(UTC\). To prevent this type of issue in the future, we are making improvements to our process for similar backend updates. No data loss occurred as a result of this issue.
