    Update: Our engineers have identified and resolved the issue in Formstack Forms that was causing intermittent application outages. We are continuing to closely monitor the situation to prevent any future outages.
  • Time: Dec. 14, 2022, 6:17 p.m.
    Status: Monitoring
    Update: **Notice:** We have identified an issue in Formstack Forms that prevented users from logging in to the Forms application as well as accessing and filling out forms intermittently. We sincerely apologize for this interruption and are working hard to understand the cause of this outage.

    Update: We are currently investigating the Formstack Forms application error of 502/Bad Gateway

    Status: Resolved
    Update: Resolved 1:45pm - 2:25pm ET Forms Dashboard screen is blank upon login. When attempting to log in customers were intermittently receiving a blank page after signing in to their Forms account. Form submissions were not impacted during this time.

    Status: Resolved
    Update: SMS activity reports over the last business day indicate service has been effectively restored with messages delivered as expected. This issue is now resolved.
  • Time: Nov. 10, 2022, 5:20 p.m.
    Status: Monitoring
    Update: Our SMS provider has identified an issue in their system preventing the delivery of Sign SMS messages to US carriers. The provider has indicated the issue has been resolved and SMS message delivery restored. Our teams will continue to monitor SMS activity over the business day to confirm this issue is fully resolved.
  • Time: Nov. 8, 2022, 4:32 p.m.
    Status: Investigating
    Update: We're aware of an issue where participants are not receiving a text notification when using the SMS delivery feature on Sign. We're working to resolve the issue and apologize for any inconvenience.

  • Time: Nov. 4, 2022, 6:30 p.m.
    Status: Resolved
    Update: From 8:56 - 9:38 am ET, 25% of login attempts may have experienced a 500 Error when attempting to log into a Forms, Documents, or Sign account. Attempting to log in again would have been successful. The issue has been resolved at this time and no action is necessary.
