Description: **Root Cause Analysis** **What Occurred?** On Thursday, April 27 an IRS tax file load was run against data from a different tax source causing existing organizations verified against the IRS to no longer be verified. This error prevented workflows from being completed in CyberGrants and new IRS organizations from being added to the database. **Why Did This Occur?** Initial preparation for the IRS tax load was scheduled to load on an earlier date, paused and rescheduled for April 27. Upon initiating the process again, a reverification process did not occur. **How Will CyberGrants Prevent Further Occurrences?** Procedures have been updated to include specific validation of data loads before any tax process is kicked off. Notification emails on potential data discrepancies will automatically hold job from being queued. Code is being reviewed and updated for improved error validation.

Description: **What Occurred?** On March 22, 2023 we identified an issue with the March CGDS invoice and disbursement process. To minimize downstream risks to payments and funding, we paused key parts of CGDS processing. **Why Did This Occur \(Root Cause\)?** The issue occurred when an update to client invoices triggered additional unintended updates to other invoices and processes. **How Will CyberGrants Prevent Further Occurrences?** We have and will make many updates to CyberGrants to eliminate the possibility of updates triggering additional issues. In the unlikely event of future issues, we will build in capabilities to isolate the core issue from CGDS processing so that other invoices, payments, and disbursements are not impacted. We will also build additional capabilities to closely monitor exceptions and status throughout the full CGDS process lifecycle.
